West Oaks Hospital has been faithfully serving Houston and the surrounding communities for over 30 years. West Oaks, located in southwest Houston, is a fully accredited 144 bed acute care facility and 16 bed adult CD residential facility dedicated to the provision of quality care and assistance for individuals and families in need of psychiatric care and treatment for substance abuse. West Oaks Hospital offers the community a broad range of inpatient, partial hospitalization and intensive outpatient services for children, adolescents and adults. We also offer residential adult chemical dependency services. West Oaks Hospital is available 24 hours a day, seven days a week for any questions or inquiries about our programs.

West Oaks Hospital is seeking a dynamic and talented Case Manager to join our team of compassionate, dedicated professionals. Purpose: The Case Manager contacts external case managers/managed care organizations for certification and recertification of insurance benefits throughout the patient’s stay, and assists the treatment team in understanding the insurance company’s requirements for continued stay and discharge planning. The UR Coordinator is responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team. The Case Manager advocates for the patient’s access to services during treatment team meetings and through individual physician contact.  The Case Manager participates in treatment team meetings and continued stay reviews as indicated.

Job Requirements

EDUCATION
Required Bachelor’s degree in human related field.

LICENSURE/CERTIFICATION
Required CPR certification and HWC certification. Preferred Licensed as a Registered Nurse in Texas.

EXPERIENCE
Required one year of case management experience and one year inpatient psychiatric experience or comparable nursing experience and demonstrated proficiency in psychiatric care.
